To book an appointmentTo book an appointment for your patient, please fax your patient's referral form to 905-948-8358.
 Referral Form
Within 2 business days of receiving the fax, we will contact your patient directly to:
- Schedule an appointment
- Provide preparation instructions
- Provide helpful material that answers common questions about the procedure
Examination office hoursMonday to Friday, 8 am - 4 pm
Follow UpWe are committed to keeping you as an active participant in your patient's care. Within 5-7 business days we will provide you a follow-up letter describing the findings of the procedure.
Personal TourIf you would like a personal tour of our facilities prior to referring your patients, we would be pleased to arrange your visit.
Questions?If at any time you have questions or concerns about your patient’s care, we want to hear from you. Please call us at 905-948-9119 between 8 am and 4 pm, Monday through Friday.
